The USA’s Ryan Murphy took the gold adding to his 100m gold to claim the double.
The Aussie swam 1:53.96, just 0.8 seconds slower than his personal best in holding off Russian teenager Yevgeny Rylov.
The Americans have won this event the past six times.
Larkin was the world champion in the 100m backstroke but could only finish fourth on Monday in Rio.
