The former Eels and Roosters player has not been seen since about 9.30pm on Saturday night when he left his Beaumont Hills house in his silver Subaru Outback.

He does not have his phone or wallet with him.

"If anyone comes across him or the car, could you please contact any of the family or the police," she wrote on Facebook.

The 36-year-old father of three won the 2002 NRL Premiership with the Roosters.

A NSW Police spokeswoman said police had received a missing person report.
