George Burgess faces a four-game ban after being cited for an eye gouge on New Zealand captain Dallin Watene-Zelezniak in England's second Test victory.
England defeated the Kiwis 20-14 at Anfield on Sunday.
In the second half, Burgess' fingers came into contact with Watene-Zelezniak's eyes.
The South Sydney prop has been summoned to appear at a disciplinary hearing on Tuesday evening.
The Match Review Panel determined that the incident merits a Grade D charge of "behaviour contrary to the true spirit of the game", and the case will be heard by a three-man panel, including one representative each from England and New Zealand.
After the match Watene-Zelezniak was sure Burgess's action was accidental.
"I’m sure he didn’t do it on purpose,” the Penrith fullback said.
"Him and his brothers are good guys and they’ve done really good things for the game. I don’t think that’s in their kit bag but it is what it is, you just have to deal with it.
"He’s a big boy and he’s got some big fingers too. I think his whole hand would probably cover my whole face. I’m pretty sweet now. The doctor gave me some kind of eye drops. I’m sure it was accidental."
